Community services provide essential care to many people, families and communities from health promotion to end of life care. This care is provided in many settings, at critical points in people’s lives, and often to those in vulnerable situations.
The Transforming Community Services (TCS) programme supports the vision of the Government’s White Paper, Equity and Excellence: Liberating the NHS, to put patients at the heart of the NHS, and provide them with accountability, more choice and better control over their care. It supports the new focus on public health through universal services; especially for children and families, and in promoting self care and early intervention.
